What a closed-loop referral status should show a partner
The minimum timeline a discharge planner will actually look at.
Five states, one sentence each
Received means you have it. Contacted means you reached the person or the family. Assessed means you know whether you can take the work. Accepted or declined is the decision. Service started means the first visit happened.
Anything finer belongs in your clinical system. Partners use the timeline to answer a family. They do not need your internal task list.
Notifications stay boring on purpose
A text or email can carry the reference code and the new state. It cannot carry the reason for a decline if that reason is clinical. The partner portal is the place for the timeline. The message is the tap on the shoulder.
CareLoop's notification template is built that way, and a test fails if patient fields leak into it.
