Home care referral growth
Dana runs a 40-caregiver agency in Texas. The work is discharge planners and SNF case managers, not another ad account.
Most new clients still start with someone who already knows the family: a hospital discharge planner, a skilled-nursing admissions lead, a physician office. CareLoop keeps that book on a map, shows which relationships have gone quiet, and gives the partner a portal so a referral does not vanish into a shared inbox.
Who actually refers
Hospital discharge planners
They need a same-week yes or no and a status they can read back to the family.
SNF admissions and social work
Short-stay buildings send personal-care cases when they trust your capacity.
Physician offices
A named liaison beats a brochure left at the front desk.
Insurer and care-management desks
Useful once you can show response time, not just a rate sheet.
A playbook for the first month
1. Draw the service area
ZIPs or a radius. The map only shows partners inside the area you can actually staff.
2. Work a short list
Identify, research, then book a 15-minute introduction. Do not spray a hundred cold emails.
3. Close the loop
Received, contacted, assessed, accepted or declined, service started. The partner sees the same timeline.
4. Read the relationship
Health score uses recency, touches, referrals in 90 days and whether they reply.
Do you replace my EHR or EVV?
No. CareLoop is the growth layer and the partner-facing status. Clinical documentation stays in the system you already use.
Can a liaison use this on a phone?
Yes. Visit logging, the map and check-in are usable on a phone. The dense table is better on a laptop.
Where does the data live?
A US workspace is meant for us-east-2. This staging host uses synthetic records only.
