Referral source finder

See who can refer near you.

Search a ZIP, postcode or city and see the organisations that can send patients your way.

Try Toronto. United States results are from NPPES and CMS. United Kingdom results are from NHS ODS. Canada is a labelled sample. The map shows organisations we could place. A free account opens the rest of the names.

Who refers

  • Hospital discharge. They refer the agency that responds and reports back.
  • Family physicians. A relationship with the office, logged visits, a health score.
  • Retirement residences. One building can be worth more than a month of ads. Treat it as an account.

Payer context

Not a claims file. CareLoop does not check eligibility.

  • Provincial home care. Publicly funded hours run through the provincial system: Ontario’s Home and Community Care Support Services, British Columbia’s health authorities, Alberta continuing care, and the equivalents elsewhere. CareLoop does not receive that queue and does not replace Ocean or Caredove.
  • Private pay. Families buy hours the public plan does not cover. The referrer is often discharge, a physician, or the residence. CASL still requires a recorded basis before a sequence.
  • Insurance top-up. Some extended-health plans contribute. The clinic or agency bills the plan. The desk only stores the revenue figure you type.

Volume, as a planning range

Planning range, not a CareLoop result. A hospital discharge office in a large city coordinates many home-care starts a week across public and private providers. One private agency on the list may receive a few private-pay or top-up cases a week when the family asks for a named company, and none from the public queue. A retirement residence can be worth more than a month of ads if the director knows who answers. These are planning ranges, not customer results.

Compliance

PIPEDA, and PHIPA for Ontario health information, need a named privacy contact, access handling and breach records. This product stores the referral encrypted and audits reads. It is not a completed privacy impact assessment. French-Canadian localisation is a later release; the type system already loads latin-ext.

Private-pay hours after discharge

Hello — we staff private-pay and top-up hours. We do not receive the provincial home-care queue in this product. A status can go back to you without the chart. CASL: we only email where a basis is on file. Fifteen minutes if useful.

  1. Week 1. Name the approver, the cases you decline, and the radius you can staff. Pull a real list from the source finder for home care and keep only the buildings you will visit.
  2. Weeks 2 to 4. 25 introduction attempts a week, split 3 hospital, 7 skilled nursing, 7 physician group, 7 case manager. Skilled nursing, Physician group, Case manager are thin in the book you already have, so those types take the larger share.
  3. Weeks 5 to 8. At a 18% reply rate that is about 5 meetings a week. Record met, no-show or rescheduled. Google and Microsoft calendars are not connected.
  4. Weeks 9 to 13. Every referral gets a status the partner can read. Type the revenue figure or the overview stays at zero. The illustration puts new active sources near 7.3 and incremental referrals near 4.1 a month.

ROI calculator

Your patient value, your referrals, three ways to pay for the work.

Sliders drive the chart. The comparison is the Growth plan, a liaison cost you edit, and the done-for-you location fee. Illustration, not a guarantee, and not a salary survey.

Assumptions you can edit

Cost used for payback is the Growth plan, annual equivalent: $274.17 per month. Change nothing about referral fees — there are none.

Questions and playbooks

Search the answers. Tick a playbook.

6 matching notes. The full set is on the guides page.

  • Do you charge per referral?

No. Starter, Growth, Scale and the agency add-on are flat fees. A test reads the pricing module and fails if a referral count is part of the maths. Seats and the size of the partner book are the limits.

  • What is in Starter?

Two seats, up to 250 partners, the map, kanban and table, booking links, the partner portal, and the overview. Discovery sequences are on Growth, not Starter.

  • What does Growth add?

Five seats, up to 2,000 partners, partner discovery in the workspace, email, SMS and call-task sequences, a consent check before enrollment, and approvals. This is the plan the calculator uses for its cost line.

  • What does Scale add?

Fifteen seats, no partner cap, and the published items for multi-location service areas, API keys, webhooks, SSO and a custom domain on the partner portal. Those Scale items are on the price list. They are not switched on in the app you can click through today.

  • What does a seat mean?

A person who logs into the workspace: an owner, a manager, or a liaison. The partner who uses the portal is not a seat. Starter is 2, Growth is 5, Scale is 15.

  • What happens at the partner cap?

Starter stops at 250 organisations and Growth at 2,000. The cap is the size of the book, not a bounty. Scale is listed as unlimited. The cap is not a fee per partner.

What changed

Full changelog
  • Source finder queries live registries

A US ZIP or city now asks NPPES and CMS Provider Data. A UK postcode asks NHS ODS. Canada stays on a labelled sample directory. Counts are what those files returned, cached for six hours.

  • Planner, tour and calculator

The 90-day plan, the screen tour and the ROI chart all run from the numbers you move. Projected referrals are an illustration.

  • Workspace you can click

Harborlight still has the partner book, sequences in the sandbox, booking, referrals and the partner portal. Email is not delivered on this host.
